With written request, and with reasonable notice, Neurological Associates will provide a patient or designee, with a copy of the medical record. Neurological Associates reserves the right to charge for the duplication costs involved in copying medical records, based on the Montana State Law 45CFR 164.524(c)(4). Neurological Associates will respond within 10 days of the date of request for all onsite charts. If the record is offsite, Neurological Associates will respond within 20 days.
