Neurological Associates, P.C.are participants with many insurance plans. However, it is your responsibility to contact your insurance provider to determine if our services are covered. When complete insurance information is received, our business office will file your claim with your insurance provider.
The following information is needed to file your claim:
- Copy of medical insurance cards.
- Insurance authorization or a written referral if required by your insurance provider.
- Subscriber name and date of birth.
- Employer if applicable.
