For your first appointment, please bring the following information:
- A list of all medications you are currently taking.
- Medical records for your current condition from other physicians.
- X-rays and/or tests results for your current condition.
- All medical insurance cards.
- Insurance authorization or a written referral if required by your insurance provider.
Please arrive 30 minutes early for your scheduled appointment to complete patient demographic forms.
